Innovative Models for Delivering Care: Supporting efforts to increase access to care within the current health care system

Along with our funding support for organizations that seek to reform the health care system, Northwest Health Foundation funds organizations and initiatives that work to redesign and improve health care delivery within the existing system - reformed or not.

In 2009, we provided grants to organizations that worked toward the primary care home model of implementation — in some cases nested within a local community health collaborative — with a focus on incorporating dental or mental health services and/or reducing health disparities

In addition, we provided technical assistance, sponsorships, and outreach to organizations involved in system redesign.
